This was a good summary of the engineering disaster. One of the lessons learned was before the disaster that there was a clear indication of problems with the bowing of the gussets, which were plain to see, but were never headed.

Well laid out utilizing the actual accident analysis for a majority of the information. A bit disconcerting that one of the lessons learned is that hand calculations, analysis software, and standards can not be fully relied upon by an engineer.
