I am personally glad that this course was thought out and presented. Although I had left the company prior to the incident, I knew and worked on the shift with both operators who were fatalities in this incident. As with most companies unfortunately, the NSPE Code of Ethics is not re-emphasized enough in industry where most of the engineers or their management are not PE’s.
