Good overview of the ethics of responsibility. It gives a clear and concise basis of the incident without the inflaming judgement on all parties. While very tragic, it has provided a basis of further process safety responsibilities and the impact of taking shortcuts. Being very familiar with the manufacture of MIC, it was always a shock to remember the the number of missteps and intentional cost-cutting actions long before the incident which should serve every PE to hold tight to the Code of Ethics
